I can't help John get through the website, but on his other question, 
how this would work with seed exchanges: A US person wishing to order 
seed from another country sends one of the labels supplied with their 
Small Lots permit to the supplier (including, e.g., the AGS seed 
exchange), who will attach it to the parcel. This label notifies APHIS 
that the contents are being imported under permit. A US organization 
wishing to receive seeds from other countries can also obtain a Small 
Lots permit and send a label to each foreign donor for their use. As far 
as I know, the US-based PBS exchange hasn't yet obtained a permit.
